Question Form Students will use the Career Interest Survey Form to establish areas of interest. From these areas, they will view a PowerPoint presentation that will explain the process of formulating the Essential Question, the precursor to the paper topic. Students will then submit for approval the Essential Question Topic Approval Form. |

| Mentor Role
and Responsibility Form This form will be given to all sophomores. The form explains the role of the mentor within the process of the development of the paper and the product. It is the responsibility of the student to secure a mentor in conjunction with the research topic of their choosing. The form does not have to be submitted until the junior year when the mentor has been secured. The form will then be submitted to the eleventh grade English teacher for placement in the Portfolio. |

Advisor Contract This form will be distributed to all sophomores. The contract explains the role and responsibilities of a faculty member who will act as an on-site advisor to the student. As with the mentor, it will be the responsibility of the student to secure the advisor. |
